What is geriatric care management?+
Geriatric care management is the professional coordination of healthcare, social services, and daily living support for elderly adults. At American Healthcare Coordination, our geriatric care management services pair your aging parent with a dedicated healthcare coordinator who handles medical logistics, insurance navigation, medication tracking, and family communication — so families managing care from out of state have a trusted partner on the ground. Unlike traditional geriatric care managers who may charge $150–$250/hour, our flat monthly plans make professional care management accessible.

Getting Started

How do I get started?+
Schedule a free 30-minute consultation. We'll learn about your parent's situation, identify your biggest pain points, and explain exactly how a coordinator would help. There's no obligation — it's genuinely just a conversation to see if we're the right fit. If you decide to move forward, we'll begin the intake process and match you with a coordinator within 72 hours.
Is there a contract or long-term commitment?+
No. All plans are month-to-month. You can cancel anytime — we'll help transition care records back to the family. Most families stay with us because they see the value, not because they're locked in.
Can I try the service before committing?+
The free consultation is your opportunity to evaluate whether we're the right fit. We don't offer a free trial of the coordination service itself — because coordination requires building your parent's care profile, matching you with a coordinator, and setting up your dashboard, which is real work that starts on day one.

How It Works

How is a coordinator assigned to my family?+
We match based on three criteria: your parent's primary health conditions (so we pair you with a coordinator who has relevant experience), geographic location (so the coordinator understands local providers and resources), and communication style preferences. You'll have a video introduction call with your coordinator before anything begins — if it doesn't feel like the right fit, we'll rematch you.
What does the healthcare coordinator actually do?+
Your healthcare coordinator manages every aspect of your elderly parent's care logistics: scheduling and tracking appointments, calling providers on your behalf, translating discharge instructions, tracking medications for chronic care management, navigating Medicare and Medicaid, coordinating family communication, and monitoring recovery milestones. They do not provide medical advice, diagnosis, treatment, legal advice, or hands-on care. Think of them as your family's geriatric care management partner.
How often will I hear from my coordinator?+
It depends on your plan and your parent's needs. Family Care plans include weekly status updates. All plans include on-demand communication — you can call or message your coordinator whenever you need them. During high-risk periods (post-discharge, medication changes), your coordinator may check in daily. Comprehensive plan families get daily monitoring during active recovery phases.
Is this a call center — will I talk to someone different every time?+
No. You get a single, dedicated coordinator who knows your parent's entire story. Same person, every call, every time. They're your family's coordinator — not a rotating pool of agents reading from a screen.

Services & Scope

What do you NOT do?+
We are administrative coordinators, not clinicians. We do not: provide medical advice, diagnosis, or treatment; give legal or financial advice; provide hands-on personal or nursing care; or assume guardianship or decision-making authority. We coordinate the logistics, advocate with providers, and organize the information — but clinical decisions always remain with your parent, your family, and your healthcare providers.
Do you coordinate care for people in assisted living or nursing homes?+
Yes. Our coordination is valuable in any care setting. In a facility, we monitor the care being delivered, attend care plan meetings, communicate with facility staff, and keep the family informed — providing an extra layer of oversight and advocacy that facility staff alone can't offer.
Can you help with two parents?+
Yes. The Additional Family Member add-on ($149/mo) covers coordination for a second parent with the same dedicated coordinator and shared family dashboard. Each parent gets their own patient database, care plan, and medication tracking.
Who manages care for elderly parents when I live out of state?+
That's exactly what we're built for. We specialize in elderly parent care from out of state — it's our core use case. Your dedicated healthcare coordinator handles everything remotely: calling providers, scheduling appointments, coordinating with local resources, and managing chronic care — so you don't have to be there in person. Distance is not a barrier to effective geriatric care management when you have a professional coordinating on the ground. We serve families across the United States.

Security & Privacy

How do you protect my parent's health information?+
All patient data is encrypted at rest and in transit. Our platform is HIPAA-compliant, with role-based access controls so only authorized family members can view health information. Each family member gets access to only what they need. For full details, see our Security & HIPAA Compliance page.
Who can see my parent's information?+
Only your dedicated coordinator and the family members you authorize. You control who has access and at what level. We never share patient information with third parties. Our administrative staff do not have access to individual patient records — only your assigned coordinator does.
Can I share the dashboard with other family members?+
Yes. You decide which family members get access and what level of information they can see. Many families give full access to the primary caregiver and calendar-only access to extended family. Access can be adjusted at any time.

Billing & Insurance

Do you take insurance, Medicare, or Medicaid?+
Geriatric care management and chronic care coordination are not currently covered by Medicare or most private insurance plans — it's a recognized gap in the system. Our service is private-pay. Some Health Savings Accounts (HSAs) and Flexible Spending Accounts (FSAs) may cover care coordination services; check with your plan administrator. We can provide documentation for reimbursement if your plan allows it. While we don't bill insurance directly, our Medicare & Medicaid navigation services help your parent maximize their covered benefits.
Can I switch plans later?+
Absolutely. Many families start with Family Care and upgrade to Comprehensive if their parent's needs become more complex. You can upgrade or downgrade at any time, effective the next billing cycle. There are no fees for switching.
What payment methods do you accept?+
We accept all major credit and debit cards. Billing is monthly, on the same date each month starting from your sign-up date. We can also accommodate annual billing upon request.
